Coccidioidomycosis is the infection caused by the dimorphic fungi of the genus Coccidioides. Primary pulmonary infection is caused by inhalation of arthroconidia. The clinical expression of disease ranges from self-limited acute pneumonia (Valley fever) to extrathoracic disseminated infection

Diagnosis
Serologic testing that is performed as part of screening (eg, immunocompromised hosts) or as part of the evaluation of a pulmonary lesion.

Serologic tests using EIA for IgM and IgG should be ordered first, if possible.
Immunodiffusion tests are generally performed to support the diagnosis when an initial EIA is positive.

Immunocompetent patients with disseminated coccidioidal infection generally have complement-fixing antibody titers of ≥1:16.

The diagnosis should be confirmed by identifying Coccidioides in one or more sites outside the chest.

The diagnosis of coccidioidal meningitis requires a CSF sample. In addition to lymphocytic pleocytosis and hypoglycorrhachia, the diagnosis is usually established by identifying anticoccidioidal antibodies in the CSF. Isolating Coccidioides from CSF is uncommon.
