Medications/Orders
0.9% sodium chloride, 30mL/hour, IV, TKO
0.9% sodium chloride, 10 mL PRN, IV, for line maintenance
2g cefepime in 100 mL dextrose 5% in water, 2g every 8 hours, IVPB, may infuse over 30 minutes.
Acetaminophen, 650 mg every 6 hours PRN, oral, for fever or mild pain

Ceftriaxone in D5W, 2g every 24 hours, IVPB, infuse over 30 minutes
Central Line Placement, stat, resident to place left double lumen internal jugular line
CBC with differential, stat

Insert indwelling urinary catheter, stat
Lactated ringer's, IV, 30 mL/kg over 1 hour, repeat if continued SVP < 90

Norepinephrine in D5!, IV, begin if SBP <90 after fluid bolus started. Norepinephrine 2 mcg/min IV, titrate to a maximum of 12 mcg/min to maintain SBP >90.
Oxygen nasal cannula per protocol, PRN, to keep sats > 92% or for increased WOB

Urinalysis, stat, do with foley catheter insertion prior to antibiotic if possible but do not delay antibiotic administration if unable.
Urine culture and sensitivity, stat, also do with foley.
