CAP treatment (Beaumont guidelines): CURB-65: 0-1: Mild treatment: PO amoxicillin or clarithromycin or doxycycline, 2: Moderate treatment : IV amoxicillin + PO clarithromycin, 3-5: Severe: IV co- amoxiclav + PO clarithromycin --> May need to alter if: Patient is admitted to ICU, Risk factors for Pseudomonas aeruginosa infection, Risk factors for antibiotic resistant organisms
78 y/o smoker with CAP, Day 2 of admission: Day 2 IV co-amoxiclav/ PO clarithromycin, Doing well / fever settled, Sputum culture grew Streptococcus pneumoniae, Blood cultures sterile, Day 3 of admission: Day 3 IV co-amoxiclav/ PO clarithromycin, He looks much better! Temperature still normal, Feeling better, eating and drinking, Sputum culture update: Streptococcus pneumoniae susceptible to penicillin, Blood cultures still sterile
