Teacher notices some signs a student is struggling.

Background information is sought
Including relevant information on diagnoses, health issues, home and family situation, bullying incidents etc.

Student is observed
The school counsellor and, if necessary, a member of the school administration observe at least one lesson and make detailed notes on signs the student is struggling.

Counsellor, teachers and administration meet
They discuss the issues and aim to identify underlying causes of the student's struggle.

Strategies are identified
These include strategies for differentiating instruction, adapting the learning environment, managing emotions and activities for home.

Parents are invited to a meeting
If appropriate, the student also attends. The concerns of the school and the strategies are discussed openly, sensitively and in collaboration.

Strategies are implemented
Both at home and at school.

Strategies are reviewed
Counsellor, teachers, parents and administration review together the effectiveness of the strategies that have been implemented, with regards to both academic and social / emotional outcomes.
