Depending on severity, immediately pain, initial instability with swelling to follow.
Usually quite clear from MOI, however need to assess extent of damage and which structured (triad).

Inability to bear weight immediately after injury and in the emergency department (4 steps) regardless of limping
Lachman's test. Instead one hand behind tibia, one on femur. Have thumb on tib tuberosity and draw forward.
Anterior draw test. Draw tibia ant - positive if lack of end feel or excessive translation compared to other side. Check sag first (posterior pos) as this can give false positive.

Objective

Observation: swelling, guarding, abnormalities, alignment change.

Pathology

Can be present with both, MCL and meniscal injury. This is known as O’Donohue’s Triad.
Graded 1 to 3. Stretch, partially torn and completely torn.
Treatment
Phase 1: regain normal joint function - ROM (reaching full extension), decrease swelling, normal PTF motion, selective quad control and normal gait. Weeks 3-4, safe to train in flexion angle of 0-60 degrees.
Phase 2: Perform specific sports activates, perform physically demanding work. At week 8 can train in flexion angle of 90 degrees.
