Intrapartum FHR monitor - Coggle Diagram

- - - - Gradual decrease in FHR with onset of deceleration to nadir >30 seconds. Onset of the deceleration occurs after the beginning of the contraction, and the nadir of the
        contraction occurs after the peak of the contraction.
      - Found in mother had low low blood pressure, high blood pressure and had anemia during pregnancy etc.
    - - Before 32 weeks >10 bpm above baseline for >10 seconds
      - After 32 weeks >15 bpm above baseline for > 15 seconds
      - Prolong acceleration high from baseline for > 2 min, but < 10 min. If greater than 10 min, it may be a change to baseline.
      - It is found during pregnancy and delivery, indicating that the baby is healthy.
    - - Average baby heart rate changes from 0-5 bpm in a period of 10 min, lasts at least 10 min.
        
        Bradycardia FHR < 110 bpm
        
        Normal FHR between 110-160 bpm
        
        Tachycardia >160 bpm
    - - FHR graph gradually decreased and returned to baseline, correlated with uterine contraction
      - Occur head compression and stimulating parasympathetic
    - - Abrupt decrease in FHR of > 15 beats per minute measured from the most recently determined baseline rate. The onset of deceleration to nadir is less than 30 seconds. The deceleration lasts > 15 seconds and less than 2 minutes. A shoulder, if present, is not included as part of the deceleration
      - Occur change position of mother, umbilical cord compression, the umbilical cord is wrapped around the neck, umbilical cord prolapses, the baby moves in those with little amniotic fluid
    - - least 15 bpm of FHS drop below baseline.
      - at least 2 minutes, but less than 10 minutes, from the beginning of the low to the return to normal
      - Occur CPD, fetal malposition, uterine dysfunction
    - - Grades of fluctuation are based on amplitude range (peak to trough)
        
        Minimal : Had change 0-5 bpm
        
        Moderate : Has change 6-25 bpm
        
        Absent : Amplitude undetectable
        
        Marked : Has changed more than 25 bpm

      - Uterine hyperactivity
      - Maternal hypotension
  - - - Late Deceleration
      - Variable Deceleration
      - FHS > 160/min or < 110/min
      - Milld meconium stained
      - Moderate meconium stained

  - - - Lying on the left side
      - On oxygen 4-5 LPM
      - Stop medication oxytocin
      - Correction of hypotension
      - Load IV following treatment plan
