Inspection
contour
symmetry
skin color
presence of pulsations
scars/lesions/wounds
tube drains/ostomies
Auscultate
look for the presence of abdominal aortic bruit
make sure to have proper placement of the the stethoscope
bowel sounds with proper stethoscope placement in all 4 quadrants
start with the right lower quadrant, the progress to the right upper quadrant, lower upper quadrant, and left lower quadrant
typical findings: normal, hyperactive, hypoactive, or absent
Palpation
light palpation
1-3 cm depth in all 4 quadrants: RLQ->RUQ->LUQ->LLQ
deep palpation 5-8 cm depth in all 4 quadrants: RLQ->RUQ->LUQ->LLQ
assess for costovertebral angle tenderness bilaterally
Percuss
2-3 areas in all 4 quadrants
start in the right lower quadrant->right upper quadrant->left upper quadrant->left lower quadrant
look for sounds of tympany and/or dullness
Position of patient
patient should be in supine position with knees bent to aid in relaxation of abdominal muscles
