medical: chronic renal failure with implications for health and end organ damage (gfr of less than 60 and other markers: haematuria, proteinuria, for at least 3 months)
layman: If you have chronic kidney disease your kidneys are gradually losing their ability to filter waste material and excess fluid from your blood. It is a serious condition, but there are good treatments that can help slow the disease.
