indications: perennial allergic rhinitis, seasonal allergic rhinitis, vasomotor rhinitis

counseling: avoid spraying in eyes; somnolence has been reported with nasal administration; instruct patient to avoid alcohol use and hazardous activities such as driving or operating machinery until level of sedation is known; review proper instillation technique, including priming the with initial use and if not used for ≥ 3 days; blow nose prior to using; no not spray into wall separating nostrils; may cause taste disturbances that are typically transient and temporary; to prevent contamination, keep tip of nose spray clean
cetirizine (Zyrtec)

interactions: CNS depressants (opioids, bentos, alcohol; possible increase in sedation), P-gp inducers/inhibitors

indications: perennial or seasonal allergic rhinitis, chronic urticaria

fexofenadine (Allegra)
interactions: CNS depressants (opioids, bentos, alcohol; possible increase in sedation), antacids (Al or Mg containing products reduce F of fexofenadine)

indications: seasonal allergic rhinitis, idiopathic urticaria
rare ADRs: hypersensitivity, insomnia

counseling: avoid activities requiring mental alertness or coordination until drug effects are known as drug may cause dizziness/sedative effects; take suspension with water only (not fruit juices), shake well before use; ODT should be taken on empty stomach and stored in blister pack until used; place on tongue and allow to dissolve, do not crush or chew; swallow with water but not with fruit juices
fluticasone nasal (Flonase, Xhance)
common ADRs: HA, nasal irritation/burning
rare ADRs: severe hypersensitivity, glaucoma, pneumonia, secondary hypocortisolism, osteoporosis
interactions: CYP3A4/5 inhibitors, desmopressin (nasal corticosteroids may enhance hyponatremic effect of desmopressin)

indications: allergic rhinitis, nonallergic rhinitis, nasal polyps
toxicity: BMD and growth and development should be monitored; routine ophthalmologic examinations should be performed; monitor for s/s of adrenal suppression or infection

counseling: advise patients on proper administration technique for this product; instruct patients to monitor for signs of toxicity, especially adrenal insufficiency
hydroxyzine (Atarax, Vistaril)
