Type 4 - Least common. intra-substance tear of the bicep tendon with a bucket handle tear of the superior labrum.
The long head of the bicep attaches directly to the superior labrum. Hence it's involvement in this pathology.
Subjective
-
More chances of tears at age 30-50,
Common activities linked: repetitive throwing, hyper extension, FOOSH, heavy lifting.
Intermittent pain, provoked by overhead activities.
Sensations of clicking and or popping, may be unable to lay on symptomatic side.

Treatment
Strengthening posterior RC, beginning to load bicep, shoulder proprioception.
