Formative
Definition: A formative assessment is a way to adjust classroom instruction to fit students individual learning needs. These assessments are conducted mid unit or lesson, and are usually not taken for a grade
exit tickets: Students will complete a "exit ticket" that can include many different things such as, questions of the teacher, reflecting over the lesson, answering assigned questions, etc.
Classroom polls: classroom polls is considered a formative assessment, because the teacher is taking in student feedback to determine the course of the class. The teacher can use the students feedback to adjust instruction.
Diagnostic
definition: a diagnostic assessment is used as a form of pre assessment to asses what students already know before starting a new unit. This assessment can be used to change the instruction based on students prior knowledge or lack there of.
Running Records a running record is used to determine a students reading level. This assesses the students error rate, fluency, and self correction rate.
PAST phonological awareness skills test. Helps to asses a point of instruction of each individual student in your classroom.
Performance
definition: A performance assessment requires students to create/ preform a task.This is a hands on assessment and is performance based, not written.
readers theater: Students perform a story that they have read using prosody. This shows student knowledge of a story and helps them to develop fluency.
Presentations: Presenting information to the whole class is a performance assessment because students are presenting information rather than writing/ answering questions about a topic.
Interim/Benchmark
definition: interim/benchmark assessments are used to asses wether a student is meeting the learning objective that is being taught.

extended essay scored with a rubric to assess wether or not a student understands the material and is able to write an essay that meets all criteria. This is easily assessed and scored by the teacher.
Summative
definition: A summative assessment is a formal assessment usually administered at the end of a unit.These assessments are taken for a grade and have high stakes.
exam/ test: A end of unit exam would be an example of a summative assessment. the students would show their knowledge from the unit by completing the test.
Book report: a book report can be used as a summative assessment by allowing students to show what they have read.
