Pagets Disease
What is it?
✦ Chronic metabolic bone disease
✦ Abnormal bone metabolism (resorption/ osteoblastic repair/remineralization)
✦ Unknown cause (possibly environmental, viral, genetic)

How is it Diagnosed?
Clinical, Radiographic, Histopathologic
Laboratory Testing
✦ Serum alkaline phosphate level is elevated
Bodansky Units Normal=1.5-5 > Can be as high as 250!
King-Armstrong Units Normal=5-10 > Can be as high as 200+!
✦ Serum, calcium & phosphorus levels are normal
Microscopic✦ Bone trabeculae surfaced by many osteoblasts & osteoclasts
✦ Mosaic bone from prominent reversal lines as a result of resorption and deposition
✦ Connective tissue is high vascularized (overlying skin may feel warm!)
Treatment
✦ Most common Tx is Biphosphonate (1 intravenous dose can keep disease in remission for 6 years!)
✦ Osteoclast inhibitors
Complications
✦ Slowly progressive
✦ Bone fractures
✦ Malignant tumors (osteosarcoma)
✦ Rare, heart disease
