Sickle Cell Disease

mutation in hemoglobin causes it to polymerize
the mutation causes oxygen to be off-loaded from the hemoglobin molecules in the RBC
The polymers form long, rigid, rods in the RBC, giving it an abnormal, sickling shape
Hemolysis of cells and vaso-occlusion
Impaired oxygen delivery
severe pain throughout the whole body
Risk factors
Family hx
Both parents must have sickle cell gene
Manifestations
Pain 10 on a 0-10 pain scale
Muscle tightness
Adequate oxygenation 98-100% RA
LVH shown on EKG
Pharm tx
Pain meds
Tylenol
Motrin
oxycodone
dilaudid
Muscle relaxer for tightness
Robaxin
RBC stimulant
folic acid
hydroxyurea
reduce number of sickled cells
hydrate RBC
increase hgb
