Step 1: Teacher requests SST. Counselor sends teacher Student Support Record for teacher to complete.

Step 1: Data (MAP, F&P, classroom-based assessments, GL assessments) indicate student differentiation is required. Student may not be responding to classroom level curriculum and or intervention.

People Responsible: Classroom Teacher, Support Teacher (LS or ELL), Admin and Counselor. School Psychologist and Speech & Language Pathologist may be present at this stage.
