Thyroid Related Emergencies
Function of the Thyroid Gland:
Butterfly shaped anterior to trachea.
Produce hormones that regulate:
Metabolic Rate
Growth Rate
Other body mechanisms
Hypothyroidism:Deficient thyroid secretions.
Cold intolerant
Delayed tooth eruption.
Commonly on HRTherapy.
*Myxedema is a common risk.
Questions to ask a patient:
Are you taking your medication?
Is your dosage stabilized?
Do you have routine Bl work to insure adequate levels?
Hyperthyroidism:
Graves’ Disease: enlarged thyroid.
Thyrotoxicosis: Excess thyroid hormones in the blood.
Nervousness, sweating, heat intolerant.
Myxedema Coma
Causes: Undiagnosed Hypothyroidism.
Incorrect treatment.
Trauma, infection, sepsis, hemorrhage, hypoglycemia, or surgery.
PABCD Treatment:
Main goal: Normalize pt temperature, use blankets.
-Admit to hospital ASAP.
-Fatal if left untreated.
I.V. admin of thyroid hormone replacement.
Symptoms:
-Deterioration of mental state.
-Hypothermia
-Hair loss
-Facial conditions
-Cool/Dry skin
-Bradycardia
-Changes in BP
-Constipation
-Seizures
-Impaired mobility
-Hypoventilation
-Coma, which is rare.
Thyroid Storm - Life threatening emergency.
Extreme Hyperthyroid State
*Main Goal: Stabilization until hospitalization.
Causes: Undiagnosed thyroid disease, overzealous treatment of hypothyroidism; discontinuance of antithyroid medication, or trauma, infectious disease, diabetic ketoacidosis and so on.
PABCD
P: Patient is Comfortable
A/B: Maintain Airway/Admin O2
C: Monitor Pulse
D:Call EMS
Symptoms:
-Fever (possibly as high as 108)
-Diaphoreses
-Restlessness
-Confusion
-Anxiety
-Psychosis
-Gastrointestinal symptoms
-Increased systolic BP
-Tachycardia
-Widened pulse pressure
-Arrhythmias
Signs of Recovering:
Levels are stable
Signs of Deterioration:
-Coma
