Pharmacology
Nephrotoxicity of drugs
-
Several forms: Pre-renal failure, acute tubular necrosis(ACT)& interstitial nephritis
Blood flow is a balance of prostaglandins mediated vasodilation in the afferent arteriole & angiotensin II mediated vasoconstriction in the efferent arteriole
-
Interstitial nephritis
Features>rashes & eosinophilia , eosinophils in urine common
-
-
Many drugs implicated>β-lactams, NSAIDs, sulphonamides
ACT
-
Reversible , and is used to monitor renal function while pts are on these drugs
-
-
Pre-renal failure
-
↓ Renal Blood flow>NSAIDS block vasodilatory prostaglandins>pre-renal failure in high risk pts. & ACE inhibitors dilate efferent arteriole>cause pre-renal failure in high risk pts.
