Care plan :
Assess hydration of patient; airway clearance is prevented by inadequate hydration and thickening of secretions
Elevate head of bed and change positions regularly to promote chest expansion and expelling of secretions
Observe sputum color, thickness and odor and report abnormalities
Suction as indicated, this mechanically clears airway in patients who cannot cough or do deep breathing techniques
Ineffective airway clearance; nurse should promote airway patency, increase fluid intake , promote effective cough deep breathing techniques

Pathophysiology
The inflammatory excudate impairs gas exchange and the excidate plus leukocyte and mucus membrane form thick tenacious secretions
Secretions may block smaller airways and alveoli, that further impairs gas exchange.
-
The thick excudates provide an excellent medium for bacterial growth,
