RENAL PHYSIOLOGY 2 cntd
H20 handling
Descending Loop: only H20 permeable>H20 lost from tubular lumen>high concentration of substances in lumen
-
ADH triggered from Pituitary by changes in hypothalamus, angiotemsin 2, baroreceptors> Effects:Insertion of H20 channels(Aquaporins-2 in the apical), Vasoconstriction via V2 receptors, urea reabsorption
Countercurrent exchange:
Ascending limb>>Na+ Pumped out from tubule, Descending> H2O pumped out
Changes in tubular system>transferred to vessels(Vasa recta)>because vessels adapt to changes in the interstitial>hence the flow is slow to allow vessel adaptation>have osmotic concentration that corresponds to that of the interstitial

K+ Handling
-
-
-
H+/K+ Exchange in the intercalated cells>pH sensitive K absorption mechanism>on the luminal side there is ATPase transport
-
Ca2+ Handling
DCT:10% via ECaC(TRPV5) , Excretion by Vit D & PTH
-
Phosphate Handling
DCT: Excretion increased by PTH , Decreased by Vit D
-
PT: 80% Reabsorbed via co-transport(overflow mechanism)>>as long as the amount of phosphate is lower than normal , all phosphate is reabsorbed
Mg2+ handling
-
-
Fine tuning.>DT through transcellular transport via epithelial apical channels , TRTMP Channels(DCT)
Cl- handling
CT:Na independent mechanism-Cl-HCO3- exchange aided by ATPase>Cl accumulates in the cell and transported into interstitial through Cl channels
