Acute Myocardial Infarction (AMI)
Cause:
Occlusion of coronary vessels
Symptoms
Dyspnea
Cyanosis
Pulse thready
Tachycardia
Diaphoresis
Palpitations
Weakness
Severe pain in chest. May radiate to neck, shoulder, and jaw
Feeling in impending doom
PABCD
Airway and breathing: Activate EMS, Process with BLS, administer oxygen
Circulation: monitor pulse, proceed with CPR if signs of cardiac arrest are present
Position: upright/ semi-reclining
Definitive tx: administer nitroglycerin 0.4 mg every 5 minutes up to three times
Signs of recovery
Appears stable
Still necessary to transport to hospital
Pain lessens
Tachycardia
Rapid heart rate
If prolonged: tachycardia-induced cardiomyopathy
Bradycardia
Slow heart rate
If prolonged: heart failure
Signs of deterioration
Loss of consciousness
CPR fails
Cardiac or respiratory arrest
