- Hypothesis generation:
A male in his 40’s has been feeling unwell for the past two years. During the last few months, he has been getting breathless when doing any physical activity. His breathlessness aggravated in the past few weeks, causing trouble sleeping unless he props himself up. Also, he gained weight and his ankles appear to be swollen.
