Strategy: Change mindset of community about disease and health issues to encourage respect among community for privacy, choice and autonomy of the patient.
Encourage discussions and open communication surrounding patients condition.

Coordination and Integration of Care
Strategy: Enhance the communication between both the patient and circle of care - transparency in the best practices and best treatment, while giving patient autonomy over choices. Enhance knowledge of professionals to treat - local requirement.
Information and Education
Strategy: Having circle of care healthcare professionals have evidence based practices which are taught to the patient to help them make informed decisions.
Physical Comfort
Strategy: Enhance comfort of hospitals, decease wait times and constantly upgrading equipment or health centres to ensure patient comfort.
Emotional Support
Strategy: Have self-management coping taught in therapy sessions locally to ensure success. Behavioural, emotional and cognitive therapists accessible to all patients within circle of care.
Involvement of Family and Friends
Strategy: Team based treatment plans and decisions to include both the patient and also the family. Encourage communication and collaboration of healthcare teams.
Continuity and transition
Strategy: Patient directed follow-up schedules to fit with their needs and their schedule as they need help and monitoring.
Access to Care
Strategy: Employing healthcare providers locally which are needed to complete the healthcare circles of care for all patients. Create accessible transportation modes for those needing more care than is available locally - example: busing or taxis to take to hospitals or treatment centres.
