Putting off maintence leading to poor quality plant, off putting to passengers and unsafe

Condition of cabins, seating, standing and accessibility issues
Condition of operators, fatigue, work hours or demands. How is work shared between operators?
Abuse of the system, are all passengers paying fee? Is additional security required?

Passenger safety, are all people on the train safe from others? Do we need security pre boarding? Who can people go to if they are uncomfortable or witness a threat or threatening event?
Emergency response. Are all fire extinguishers tagged? Is there enough fire extinguishers? Are there enough escape routes? Will these escape paths work in all emergency situations?
Are all tracks at a clear distance from trees, roads and or wildlife, to ensure limited amount of falling trees or interaction with people and plant?
Are all bridges underneath tracks sturdy? How will it be affected if cars and or trucks impact the underpasses? Is this punishable?
