PERMIT TO WORK - Coggle Diagram

- - - - Isolation work area & confined space
    - - Draining/ Purging
    - - All authorized entrants must use the standard protection PPE;
    - - Removed unwanted gases, heat generated, dust, and smells from the space
      - Provide fresh air for the entrants
    - - Start work only after the permit is approved by Entrance Supervisor (ES) & Permit Issuer (PI)
    - - Air from forced ventilation shall be from a clean source, free from contaminants
    - - Test air at the top, middle and bottom, and in remote areas. Inspection done by competence Authorized Gas Tester (AGT)
    - - Maximum duration of PTW is 8 hours, may be extended for a maximum of 4 hours (provided all the terms & conditions remain the same)

    - - Coordination of all activities to provide a safe working environment for all personnel at the site.

      - The purpose of PTW documents is to provide guidance in confirming:
    - - Process supervisor:
        
        Delegates responsibilities to the process technician and ensures that all established procedures are completed
      - Maintenance supervisor:
        
        Inspects area and ensures that it is ready for the safety inspector; ensures that equipment, vessels, and piping are cleared; ensures that safety equipment is located near job site; reviews procedure with person performing the work; confirms PPE required; and signs permit
      - Process technician :
        
        Inspects area and ensures housekeeping
        
        Isolates
        
        clears equipment, vessels, tanks and piping
        
        apply LOTO
        
        determines PPE required
        
        fills out the permit
      - Person performing the work:
        
        Inspects the job site;
        
        gathers information from process representative and mechanical supervisor about potential hazards, special procedures, or conditions; and
        
        selects and wear appropriate safety equipment before beginning work.
      - Safety permit inspector:
        
        Inspects the area and ensures that it is safe;
        
        performing gas test and determines oxygen level;
        
        ensures that equipment, vessels, and piping are cleared;
        
        confirms required PPE;
        
        signs permit; and
        
        sets time limit. Process technicians at many locations perform this function.
      - Standby Person
        
        A person that ensures that the person performing the work is safe, wears the PPE required to perform the job, warns the person performing the work if a hazardous condition develops, and calls for help, if needed.
        
        Sometimes called a "hole watch”, buddy, or attendant.
        
        That a fire watches will be posted in place during the procedure and 30 minutes after the work is complete.
        
        The reason the fire watch remains on post 30 minutes after the work is complete is just in case an unseen spark from the hot work should ignite material in the area
        
        Fire watches are used to prevent and/or extinguish fires at the incipient (beginning) stage. They monitor the hot work sites to ensure that conditions do not change when combustible materials cannot be effectively removed, protected or shielded
