Struggling Student Referral Process -

Teacher identifies concerns through observation and assessment
Teacher communicates concern to school counsellor
Teacher and counselor meet to discuss findings on student
Teacher and counselor meet with parents to discuss concerns. Learning or accommodation plan agreed on.
Teacher begins using learning plan. Parents kept informed on student's progress.
If learning plan is effective, teacher will continue to use and monitor
Learning plan reviewed and kept on file for next grade level
Parents and counselor and next grade teacher meet to discuss adjusting and continuing learning plan
If learning plan is not effective, teacher will meet with parents and school counselor to review and adjust
Adjusted learning plan put into use and monitored
If parents do not agree to learning plan, teacher and counselor continue to monitor situation and keep records
Parents come forward and identify concerns to teacher
Learning plan is succeeding
