        85% of participating families/individuals with I/DD will be satisfied with the process, progress and level of community involvement in the work.
      - Partner organizations will be surveyed annually to track the shared measure mentioned above. Results will be reported in quarter 4 for each of the three years.
        
        A minimum of 3 partner organizations will report that they have integrated one or more training module into their work with youth with I/DD transitioning to adulthood and/or individuals in long-term residential facilities transitioning back to the community.

        85% of those completing any training module will indicate that they are satisfied with that module.

        100+ individuals who complete the SDM training will indicate that they are likely to complete a SDM Agreement sometime in the future.
        
        A minimum of 500 people across North Carolina will complete the Supported Decision-Making training in person, virtually, or by any combination thereof, by June 2023
