PRIMARY NEPHROTIC SYNDROME
MINIMAL CHANGE DISEASE
DEFINITION
A kidney disease in which large amounts of protein is lost in the urine
Selective proteinuria
Nonselective proteinuria
AFFECTED GROUP
Most common in children
AETIOLOGY
Recent vaccination
Immune stimulus
Recent infection
T cells release GPF
Damage foot processes of the podocytes
Often idiopathic
INVESTIGATIONS
Immunofluorescence
Negative
Electron microscopy
Effacement of podocyte foot processes
Light microscopy
Lipids in proximal tubular cells
Normal glomeruli
FOCAL SEGMENTAL GLOMERULOSCLEROSIS
AFFECTED GROUP
African descent
Hispanic descent
Common in adults
COMMON CLUES
Heroin abuse
HIV infection
Interferon treatment
Congenital malformation
DEFINITION
Focal: only some glomeruli are affected
Segmental: only a segment (part of the glomeruli) is affected
A type of glomerular disease and describes scarring (sclerosis) in your kidney.
AETIOLOGY
Deposition of lipids & proteins in the glomerulus
Hyalinosis
Sclerosis/scar tissue
Effacement of podocyte foot processes
INVESTIGATIONS
Immunofluorescence
Often negative
Sometimes positive for deposits of C3, C1 / IgM
Light microscopy
Hyalinosis
Sclerosis
