Formative
Definition: Is used in order to monitor student learning to provide ongoing feedback that can be used by instructors to improve their teaching and by students to improve their learning.
Questioning: During instruction asking questions to check for understanding can be used as a formative assessment.
Exit tickets: These at the end of the lesson are a quick and easy way to assess where the students are and how much more instruction on the topic they may meed and the points a teacher can plan for future instruction.
Diagnostic
Definition: A form of pre-assessment where teachers can evaluate students' strengths, weaknesses, knowledge and skills before their instruction.
Running records: Giving students running records to check reading levels and to track where they may struggle is a great way to catch if the student is gifted, dyslexic, or many other reading related abilities.
Word list reading: Having students read sight words on a list can be a great way to check students knowledge, levels and also assess them as to how well they read and use their skills / reading strategies.

Summative
Definition: Used to evaluate student learning at the end of an instructional unit by comparing it against some standard or benchmark.
End of unit exams: At the end of unit giving the students an exam to have them show their knowledge of the content is a great way to assess the students understanding.
Essays: Giving students an op[urtuinty to show their knowledge in essay form will allow them to show their skills in writing but also how much of the content they understand by the end of a unit or topic.
Performance
Definition: A form of testing that requires students to perform a task rather than select an answer from a ready-made list.
Graphic organizers: Using graphic organizers to have the students show what they learned in their own way is a great way to have the student be assesses while allowing them to create something using their creativity.
Scavenger hunts: Giving students tasks that they need to complete pertaining to the topics being taught. In order for them to get the end of the hunt they must complete each task collecting something that they can use at the end to complete the last task. This is a fun and interactive way to assess students where they can be competed, have fun and learn all at the same time.
