WOUND HEALING
COMPLICATIONS OF WOUND HEALING
INFECTION
.
IMPLANTATION CYST
•Intradermal or subcutaneous saclike structure, the wall of which is stratified epithelium containing keratohyalin granules.
PIGMENTATION
.
INCISIONAL HERNIA
•Present as a bulge or protrusion at the area of a surgical incision.
HYPERTROPHIED SCARS
HEALING BY SECOND INTENTION
SEQUENCE OF EVENTS
1.Initial haemorrhage
2.Inflammatory phase
3.Epithelial changes
4.Granulation tissue
5.Wound contraction
6.Presence of infection
CHARACTERISTICS
•Open with a large tissue defects, at times infected.
•Having extensive loss of cells and tissues
•The wound is not approximated by surgical sutures but is left open.
PROLIFERATIVE PHASE
•2 days to 3 weeks.
Granulation
•Fibroblasts lay bed of collagen. They fills the defect and produces new capillaries.
Contraction
•Wound edges pull together to reduce defect.
Epithelialization
• cells spread to moist surface. Cell travel about 3 cm from point of origin in all directions
INFLAMMATORY PHASE-Immediate to 2-5 days.
Inflammation
-Vasodilation is initiated by histamine, followed by phagocytosis of pathogens by the macrophages
Hemostasis
- occur in the moment that the injury occurred. Vasoconstriction takes place to stop the bleeding. Blood clotting process is accomplish by platelet aggregation and thromboplastin clot.
HEALING BY FIRST INTENTION
SEQUENCE OF EVENTS
Initial haemorrhage
Acute inflammatory response
Epithelial changes
Organization
CHARACTERISTICS
Clean and uninfected
Surgically incised
•Without much loss of cells and tissues
•Edges of wound are approximated by surgical sutures
REMODELLING
•3 weeks to 2 years
•New collagen forms which increases tensile strength to wounds.
•Scar tissue is only 80 percent as strong as original tissue.
