Yes/No questions:
Formula: Had + subject + past participle verb + complement?
Examples:

Wh questions:
Formula: Wh + past tense of verb to have + subject + past participle verb + complement?
Examples:
When had I gone?

Simple Present
WH questions
Examples:
3rd person in singular: Where are they from?
3rd person in singular: Where do you work?
Formula: Wh + do/does + base verb?
Yes/No questions
