CONGESTIVE HEART FAILURE
PATHOPHYSIOLOGY
-
can occur acutely, with onset over several hours or days, or it can be chronic, with onset over months to years.
-
INTERVENTION
Change position frequently. Elevate feet when sitting. Inspect skin surface, keep dry, and provide padding as indicated.
-
Provide small, frequent, easily digestible meals.
-
MEDICATION
-
Beta-adrenergic blockers improve survival rates by decreasing the heart rate, reducing the work of the heart, and lessening the O2 demand of the myocardium.
ACE inhibitors decrease preload and afterload by blocking the RAA system, resulting in vasodilation, decreased blood volume, and lower blood pressure.
Inotropic agents such as dopamine may be prescribed initially for short-term management to improve cardiac contractility, improve renal perfusion, and decrease fluid retention.
ASSESSMENT FINDINGS
-
-
-
assess heart sounds, rate, and rhythm.
PATIENT EDUCATION
Excess weight makes your heart work harder. Your health care provider may prescribe a weight loss diet for you.
-
-
Weigh yourself each morning, before breakfast, on the same scale and wearing the same amount of clothing.
