Module 4: Mechanical Ventilation (Pre-mindmap)
Overview
A complex area that PT has a role in managing; from airway/secretion management, to early mobilisation.
Part of a complex MDT; extended scope PT can have more detailed role in management of Vent. systems. Role from pre-intubation to immediately post-extubation
Knowledge of pathophysiology, cardiopulmonary anatomy, an understanding of medications and (more importantly recently) infection control procedures is crucial.
Facts
Non-invasive vent. is first line, but may not be tolerated/appropriate. 2 types: ETT and tracheostomy, length of time depends on local guidelines (ETT usually only for 1 week)
Different 'modes'; depending on patient requirements (pressure support or volume support)
Must always consider secondary issues of ICU care; muscle loss, nutrition, cognitive issues etc.
Feelings
An area I have always enjoyed working in, although sporadically in recent times.
Enjoy the level of detail required in understanding the 'mechanics' involved.
Opportunities/Benefits
Review of MHI vs MHI; traditionally at our facility we use VHI. More consideration with current infection control procedures.
In my current role in rehab- the secondary symptoms of ICU admission are certainly prevalent. Review the literature around expected timeframes for recovery (esp. with cognitive elements).
Difficulties
Have probably de-skilled slightly in recent months with a change in job role.
Have always struggled with 'flow volume loops' and how to interpret these.
Creativity
Continue to work with my clinical supervisor in maintaining these skills- despite change in job role.
Where possible, hands-on application, joint treatment sessions with clinical supervisor to maintain and further clinical knowledge.
