Uses
Used to treat hypothyroidism
Look for therapeutic efficacy (lose weight, improve tolerance to environmental temperature, increased Activity, increased pulse rate)
Treatment Overdose
Severe thyrotoxicosis: use BB's to prevent cardiac toxicity
Administration Alerts
PO 1-3 weeks to work; dose up slow in patients with pre-existing cardiac disease
Monitor TSH levels (if TSH is increased, increase dose of T4)
Adverse Effects
Palpitations, Dysrhythmias, Insomnia, Wt. loss, Heat intolerance, menstrual irregularities
Black Box Warning
USE OF THYROID HORMONE TO LOSE WEIGHT IS CONTRAINDICATED
Contraindications
Thyrotoxicosis
Severe cardiac conditions and renal insufficiency
Correct adrenal insufficiency first
Caution: patients with cardiac disease, HTN, impaired kidney fnx, older adults; if worsening diabetes symptoms- may need to adjust glycemic agents
Drug-Drug
Cholestyramine decreases absorption of levothyroxine
Epinephrine, NE: can cause increased cardiac Insufficency risk
Warfarin results in increased bleeding
