84-year-old female is 5 foot, 5 inches and weighs 165 pounds (BMI 27.5). She has a history of hypertension, hypercholesterolemia, asthma, COPD and prediabetes.
Patient was brought into the clinic because she has been very forgetful, forgetting the stove being on, doctor's appointments, even people's names
Mental assessment was done, patient has severe cognitive impairment. Testing will need to be done to confirm diagnosis of Alzheimer's .
A referral was given for healthcare. Patient lives with daughter and they are both frustrated. Nurse teaches about how music can calm patient. An environment assessment was done and nurse made recommendations.
Make sure patient has regular routine, familiar environment, limit environmental stimuli, go over dietary schedule. Months after original assessment patient develops dysphagia. The patient ends up going to a skilled nursing facility
