Dementia - Coggle Diagram

        This allows the patient to have some traction of feet due to the
        feet becoming smooth with less traction
      - Clear pathway
        
        Making sure there is a clear path in the house to places like the
        bathroom or kitchen especially at night so the patient doesn't fall
      - Throw rugs
        
        Keeping the floor clear of things that could potentially make the
        patient fall
    - - Dizziness
        
        Making sure the patient gets up slowly and has assistance walking when needed
      - Confusion
        
        Making sure the patient has assistance and a safe and comfortable environment
  - - - Making sure patient has the privacy needed
      - Making sure the patient has any assisting devices needed for dressing
    - - Making sure the patient has what they need to feed themselves
      - Placing utensils and drink where the patient can reach for them easily and assist them as needed
    - - Assist as needed but letting them do the majority of bathing/ brushing teeth for sense of independence
      - Making sure patient has what is needed and show them where it is
- - - - Mental assessment was done, patient has severe cognitive impairment. Testing will need to be done to confirm diagnosis of Alzheimer's .
        
        A referral was given for healthcare. Patient lives with daughter and they are both frustrated. Nurse teaches about how music can calm patient. An environment assessment was done and nurse made recommendations.
        
        Make sure patient has regular routine, familiar environment, limit environmental stimuli, go over dietary schedule. Months after original assessment patient develops dysphagia. The patient ends up going to a skilled nursing facility
