Worker fell off raised forklift pallet

The forklift procedure has been the company method for about 5 years

The worker was being lifted by forklift while standing on a pallet
He was being raised to clean solar panels on top of a shipping container

Cleaning the solar panels is a regularly scheduled activity

The worker placed the cleaning products there and stood on the left side
