Intracranial Regulation: Ischemic CVA
Pathophysiology:
Occlusion of blood flow in part of the brain caused by a thrombus or embolus
Cellular hypoxia occurs
Cell depolarization and cellular membrane are impacted
Blood flow decreases areas of ischemia occur
Infarction occurs
Cellular membrane permeability is effected as increase of calcium, sodium, and water can cause edema
Lack of oxygen causes neurons to die
Etiology: Can be caused by atherosclerosis, obstruction or dissection.
Risk Factors:
Over 65 years of age
Obesity
Smoking
PAD
Signs and Symptoms:
Sudden headache
Facial droop
Hemiparesis
Difficulty swallowing
Aphasia
Complications:
Death
Paralysis
Loss of cognitive function
Collaborative treatment:
Maintain airway
Aspirin therapy within 48 hours of onset
Alteplase and antihypertensives
Diagnostic/Lab Tests:
CT Scan
MRI
coagulation study
