Paramedic
Intervention
Assess Tom's health and well-being
If Tom appears dehydrated, Sodium Chloride 0.9% 250ml can be administered through the vein using IV cannula
Explain to Tom what they intend to do to help him
Tom may also be offered analgesia for pain management
Provide Tom with reassurance
Introduce themselves to Tom
Wound Care
Transportation to the hospital
Referral to safeguarding in regard to mental health if required
Adult Nurse
Intervention
Check for other signs of infection by taking urine sample and blood sample.
Provide IV paracetamol to lower temperature
Take basic observations, such as temperature and respiratory rate
Explain to Tom the care that needs to be provided
Introduce themselves to Tom
Provide reassurance
Follow the Sepsis 6 care bundle 1. Administer high flow oxygen 2. Take blood cultures 3. Measure serial lactate levels 4. Administer intravenous antibiotics 5. Administer intravenous fluids 6. Monitor urine output
Continue to prescribe Olanzapine and advise the doctor of any queries related to Tom's non-compliance, the Olanzapine can then be reviewed.
Assess Tom's mental capacity
Contact Tissue Viability team to assess Toms wound

Intervention
Introduce myself and explain my job role
Discuss with Tom what he believes is best for his recovery
Checking for potential risk factors and/or safeguarding concerns throughout
Discuss non-compliance with Olanzapine and possible side effects Tom may be experiencing, see if there are other suitable medications Tom would try
Build a therapeutic relationship
Speak to Tom about therapies that are available
Gain consent from Tom to proceed with a crisis assessment
Speak to Tom about coping strategies
Depending on risk factors and Tom's likeliness to harm himself or others, Tom many require detention under the Mental Health Act which requires liaison with a psychiatrist/doctor
Read through Tom's medical notes since admission
Possible referral to social services, CAMHS, current case worker, crisis housing, secondary care facility
