Coagulopathies (ii) - Coggle Diagram

- - - - usually mild
      - not dxed until puberty/adulthood
  - - - should be used with caution in under 1s, can cause hyponatraemia + seizures
- - - - inadequate intake
      - malabsorption (coeliac, CF, obstructive jaundice)
      - warfarin
  - - - consider congenital cause
      - BM exam to excl leukaemia/aplastic anaemia
    - - benign + self-limiting in 80%, remitting spontaneously after 6-8wks
      - most managed @ home + don't require admission
      - only tx if major bleeding or persistent minor bleeding interfering with QOL
      - can give PO prednisolone, IV anti-D, IVIG
      - platelet transfusions only with life-threatening haemorrhage
      - avoid trauma + contact sports until platelets recover
    - - in 20%
      - platelet count still low after 6mo
      - tx = supportive mainly
      - drugs only if impaired QOL - rituximab
      - splenectomy if drug tx fails
      - regular SLE screening
  - - - tx underlying cause
      - fresh frozen plasma to replace clotting factors
      - cryoprecipitate (fresh frozen plasma thawed + then centrifuged + precipitate is collected)
      - platelets
      - antithrombin + protein C concentrates
      - use of heparin controversial
- - - - AD
      - heterozygotes are thrombotic, homozygotes @ v high risk, life-threatening
