Coagulopathies (ii)
vWD
vWF has 2 roles
-
carrier protein for VIII:C, protecting it from inactivation + clearance
-
-
-
-
clinical features
-
-
mucosal bleeding (epistaxis, menorrhagia)
unlike haemophilia, spontaneous haematomas + haemarthroses uncommon
-
AVOID IM INJECTIONS, ASPIRIN, NSAIDd
Thrombocytopenia
-
< 20 = severe - risk of spontaneous bleeding - GI haemorrhage, ICH, haematuria
-
-
bruising, petechiae, purpura, mucosal bleeding
Acquired coagulopathies
-
-
-
DIC
-
diffuse fibrin deposition, consumption of coag factors + platelets
causes incl sepsis, shock, trauma, burns
-
bruising, purpura, haemorrhage
-
-
suggestive: thrombocytopenia, prolonged PT + APTT, low fibrinogen, raised fibrinogen degradation products, raised D dimers
