Allergic Rhinitis & Conjunctivitis (Rhinoconjunctivitis)
Classification
Atopic (IgE abs to common inhaled allergens) or non-atopic
Intermittent or persistent
mild or severe
seasonal (related to grass/weed/tree pollen) or perennial (related to dust mites/pets)
Intro
underestimated cause of childhood morbidity
affects up to 20% of children, can severely disrupt their lives
a/w eczema, sinusitis, adenoidal hypertrophy, asthma
Presentation
coryza (inflamm of nasal mucous membranes)
conjuncitivitis
cough due to post-nasal drip ("cough-variant rhinitis")
sleep disturbance due to blocked nose
impaired daytime behaviour + concentration
Tx
2nd-gen non-sedating antihistamines (topical or systemic)
topical corticosteroid nasal/eye preparations
NOT systemic corticosteroids - risk of adverse effects
cromoglycate (mast cell stabiliser) eye drops
LT R antagonist e.g. Montelukast
Nasal decongestants (for no longer than 7-10d due to risk of rebound effect)
allergen immunotherapy
sublingual or subcut
limited by anaphylaxis risk
