-Observations and concerns of Jean Claude's eating habits and nutritional needs.
-Altered speech patterns making him difficult to communicate with. Consulting other team members.
-Assisting him with ADLs as needed.
Dentistry
-Assess for high levels of plaque and many oral health problems related to oral soft tissues, such as gingival bleeding, periodontal pockets, stomatitis, mucosal lesions, and reduced salivary flow.
-Dental evaluation for difficulty eating or chewing. This may reveal dental issues that he is at risk for or actively has
Social Work
-Assessment with daughter and with Jean-Claude. Assess situation at home for daughter. Give her resources on how to cope with her father's state. Assessment with Jean-Claude, cognitive, depression screening.
Nutritionist
-Work with dentistry and primary team to ensure his nutritional needs are being matched with his comfort
-Speech pathology suggestions for swallowing capabilities may impact his nutritional needs. Suggestions from nutritionist for nutritional supplements that suit his needs/capabilities.

Spiritual Care Leader
-Voodoo practices may need to be considered
-Apprehension toward medical and dental practices may require education from spiritual leader.
-Help Jean-Claude build relationships with care givers and medical professionals and vice versa. Help educate health care professionals.

PT/OT
-PT evaluation for safety in his surroundings, if is dementia is worsening he may have additional needs
-OT may be able to work with and assist the nutritional and dental issues he has and help him eat more comfortably
