Pressure Ulcer Prevention: reducing/and preventing Hospital Acquired Pressure Injuries
Turn and Position
Offload weight every two hours
Prevents pressure from being on one area of the body too long
Position pillows between areas of the body that press against each other (legs or ankles)
Pressure pints can be caused by two areas of a body being pressed together
Offload heels and elbows by position pillows underneath
Bony prominences on mattress can increase breakdown and decrease circulation to the area
Use foam mattress
Foam mattress can be more comfortable and decrease pressure
Make sure sheets are not bunched up under you
Bunched up sheets can cause pressure on areas of the body
Use heel/elbow booties
Same concept as using a pillow to keep bony prominences off mattress
Float heels
Keep heels totally off bed by putting pillows under ankles
ROM exercises
Promote circulation
Barrier Cream
Moisturize daily
Dry and itching skin may cause openings in the skin
Use barrier creams on groin area to prevent breakdown from incontinence
Protects for irritation such as urine burns by keeping urine and feces away from skin
Use in places that are often moist (groin, skin folds, buttocks)
Moisture breaks down skin
Cleanse before using barrier cream
Don't trap the "bad things" underneath (Sweat, dirt, urine)
Do not scrub barrier cream off to prevent skin irritation
Can shear skin
Use around areas that may have heavy drainage
Heavy drainage on skin can cause more breakdown
Skin Integrity
Do not drag yourself up in bed to prevent skin shearing
Causes friction which may damage or tear skin
Use of turning devices such as a Prevlon
Decreases friction
Use Allevyn on bony prominences
Helping pad and protect area
Proper positioning in bed
Thoroughly dry skin folds (under breast, and stomach folds)
Rashes and fungal infections can fold in damp skin folds
Monitor bony prominences for breakdown (redness, bogginess, blanching)
Use skin prep on vulnerable areas (heels)
Creates a barrier on skin
Diet
Drink lot of water
Eat enough calories per day
Avoid malnutrition, have well rounded diet
Eat protein
Meat, fish, beans, eggs, milk
Have good intake of Zinc
Cereals, seafood, red meat
Eat food high in Vitamin C
Citrus fruits, tomatoes, peppers
Eat foods with Vitamin A
Cereals, Liver, Carrots
