Diagnostics
Stage I: Defined area of persistent discoloration (Red in light skin, blue/purple in dark skin)
Stage II: Partial thickness wound involving the dermis and/or epidermis, may present as abrasion, blister or crater
Stage III: Full thickness, damage and necrosis of subcut tissue. Deep crater where subcut tissue may be visible but not muscle or bone.
Stage IV: Full thickness skin loss and necrosis with extensive damage to underlying subcut tissue, may involve and expose muscle, bone and support structures
Collaborative Treatment
Prevention

screening
proper nutrition/ hydration
toileting if incontinent
