Obstruction occurs through some mechanism. This leads to fluid and gas build up leading to proximal intestinal dilation. As a result peristalsis increases above and below the obstruction and secretions increase. As distension increases compression of tissues occur resulting in increased hydrostatic pressure in capillary beds. This causes fluid to shift into the abdomen (third spacing) resulting in massive losses of fluid electrolytes and other blood components. F&E absorption has also decreased at the same time resulting in profound dehydration leading to hypovolemic shock. Additionally obstructions can also result in loss of blood flow leading to necrosis, perforation and internal bleeding/sepsis.
