On admission, pt was confused, experiencing hallucinations, strange dreams, very disorientated. Possible opioid overdose? Two weeks later when I next saw her, pt was lucid and her condition had improved drastically.
Possible reasons for improvement:
Hypercalcaemia treated with zolendronic acid
Humerus fracture support changed from flimsy sling to supportive plaster of Paris cast
Increased level of nursing care (query as to the timings of medication given in pt's home environment)
Changed medication, decreasing the dose of Longtec and switching to more reliable syringe driver
