Physiological Integrity
Lab values: RBC: 3.47, HGB:9.6, Hct: 27.4 this could indicate anemia, insufficient o2 circulating through the body.
Nursing d/x: Fatigue r/t low Hgb, Hct, and RBC values aeb abnormal lab values.
Short term: Pt will shower with assistance Long term: Pt will have Hgb, Hct, and RBC values WNL.
Medications: Phytonadine Inj. 1mg, help blood clot/prevent bleeding. Prenatal 0.8mg tab, 2x/day to cover nutritional gaps mom has. Cefazolom, 200mL/hr, 1 dose prior to incision to prevent infection. Ketorolac, 20mg q 6 hours, for pain
Safe Care
C section and tubal ligation, 3/23/2021
Nursing d/x: Impaired physical mobility r/t surgical incision aeb pt verbalizing pain 8/10 when moving.
Short term: Pt will have a reduction in pain scale number within 2 hours. Long term: Pt will walk to and from bathroom w/o assistance.

Short term: Pt will express feelings of anxiety to HCP. Long term: Pt will use relaxation techniques when feeling anxious.
Interventions

Advise pt to push call button when pt wants to adjust herself in bed/leave bed. This is a safety issue. Pt cannot ambulate safely on her own.
Pt will ambulate asap. This will help pt bowels, help blood flow for recovery, and help prevent blood clots.

Interventions:
Monitor pt for fatigue, paleness, tachycardia, and bleeding. This could lead to life threatening conditions.
