Overview
Physiotherapist knowledge of anatomy and function is crucial to differential diagnosis; esp. ruling out sinister causes
Well placed to provide in depth education for these patients, and to recognise when referring on to specialist (counselors neurologist etc.) may be appropriate.
Feelings
Much more confident with those that are not responding as expected- for the subacute/chronic patients.
Much clearer in interpreting assessment findings and clinical relevance (previously became overwhelmed)
Opportunities/benefits
From the case this week; felt my education/explanation was reasonable, but could improve on implementing teach-back methods to ensure pt understands.
Could look at reviewing the out of date brief handout that our dept. uses- including updating the discussion around other symptoms.
Review of the literature and vestibular migraines- something I know very little about, and no clinical experience.
A literature review for the re-admission of 'dizzy' patients to ED following discharge- interesting review from Terahni et. al. for statistics for the USA,
Creativity
Provide an in-service to our dept. to consolidate knowledge
Link with Neuro PT at base hospital to provide clinical supervision for a case study- review findings and actively reflect on assessment / treatment
Difficulties
At times can be a long period between seeing 'dizzy' patients within my clinical roles, fluctuating clinical opportunity to consolidate this enhanced knowledge
Often see people who are 'dizzy' miss the stroke pathway- ongoing education of colleagues around importance of subjective and objective assessment to distinguish symptoms
Facts
Have already implemented the further balance/vestibulospinal tests this week; with much greater understanding of how to interpret results and develop a program based on results (previously did not know about these).
Able to draw a brief overview of anatomy and function of the vestibular system- which further enhances my own understanding.
