Special Education Referral Process
5) Letter to parents
Once accepted, a formal letter is sent out to parents
To inform them that cheir child will be getting extra support at school
To inform them if there are techniques or strategies that should be done at home as well
6) Meeting with all teachers involved
Teacher, TAs, Learning Support teacher and parent
Discuss and set goals together for the student
Everyone who works with the student should be aware of the situation
2) Referral Form
Report the case to the learning support department
At any time during the year
Present evidence (assessments + sample work)
MAP tests
Reading assessments (RAZ)
Fill out an official referral form
Motor and gross skills
Observations
Literacy/ Math
Behavioral issues
What accommodations have already been done
If student needs support in Language Arts or Math
1) Observations
Teacher, TA and any other specialist's observations on student behavior in the classroom, in the playground etc.
Any signs of a struggling students
Disruptive behaviour
Not being active in the classroom
Issues with decoding
Underdevelopped phonological awareness skills
Assessments
MAP
Literacy and Math scores
Sample work from class
4) Meeting
Teacher and Head of Learning Support Department
Discuss conducted observations
Determine if request is accepted or rejected
3) Official Observations
Multiple observations conducted by a member of learning support department
Observations are done during different times of the day (beginning of the day vs end of the day)
Beginning of the day vs end of the day
Inside and outside the classroom
7) Follow up
Goals should be frequently reviewed
