Dyspnoea management

Postioning
-
-
We should discourage the patient from pushing down or gripping with their hands as this causes an elevation of the shoulders and overuse of accessory muscles
-
-
-
Why we wouldn't use it: if patients have a restrictive lung disease the leaning forward position would compress the lungs more... these patients prefer an open position
Breathing control
Breathing techniques
-
Blow as you go
-
-
Breathe in before the effort and breathe out during the effort (like when doing core exercises) - used when stepping up, standing up, lifting, bending... any that involve effort
Paced breathing
Useful during activity e.g. walking, climbing stairs
-
-
e.g. inhalation on the stair, exhalation on the step-up

Technique:
- The patient should always be in a comfortable well-supported position. you may wish to combine breathing control with positioning techniques
- the patient is encouraged to relax the upper chest, shoulders and arms
- One hand (Patients or therapists) can be positioned lightly on the upper abdomen and one on the upper chest (place therapists hand onto of patients)
- The patient is encouraged to take TV breaths at their own pace, whilst trying to move the lower hand more than the upper hand during inspiration and expiration
