Aetiology of Gout
Hyperuricemia: elevated uric acid levels / at 7mg/dl (George & Minter, 2020).
Decreased excretion of uric acid or an increased bodily production of uric acid / often due to kidney processes i.e. decreased GFR

Clinical presentation
Swelling, heat & severe pain at the site of gout inflammation in joints (CDC, 2020).
Pain may present at night; a patient may observe heat, redness and swelling at the affected joint
Generally, one joint is affected during a flare-up; commonly the largest toe (CDC, 2020). This is known as podogra.
Tophi
present as hard formations found on forearm, achilles tendon and tibial of leg; causing swelling, redness and eventually joint pain. At first presentation/formation, these tophi can be painless for the patient.
Diet: Purine nucleotides are broken down into uric acid; diets high in purines can be a risk factor for gout development
