Occupational Therapy
Adaptations to her home
i.e. bilateral rails, needs to be discussed with landlord as privately rented.
Issue betty with equiptment in the home, stools in the kitvchen/bathroom for breaks when washing/dressing due to swollen ankles, mowbray frames/bed levers, only i necessary.

Contact GP/district nurse for a home visit to assess diabetes and current medication. Look into frequent urination

Does betty feel as though she wants more support at home, e.g. a career to come in regularaly
speak with the waste removal services, can they come and help betty with binbags to reduce build up.
arrange OT home visit
Aassess bettys current living arrangements, use a risk/hazard assessment and identify issues- e.g. clear rubbish bags/trip hazards.
Help betty get a routine/schedule for domestic and household tasks, such as cleaning- these can be used as a form of exercise.
Other OT interventions that could be potentially used- Get betty into gardening- she lives on the grouond floor, this can keep her busy and be used as a form of exercis- good for physical and mental health.
educate betty on how to pay bills, set up direct debits maybe.
Friends and family
Inform Betty, her brother and her friend on her health conditions and discuss with them ways they could help Understanding her conditions fully may help them when interacting with betty; e.g. does her friend know about her diabetes and what she should be having in her meals. must get consent from betty to share this info.
Arrange regular skype/zoom calls with her brother to reduce bettys isolation, with consent from both parts.
Mental Health
Sophie's View
Main priority is to ensure betty is clear of any infection. Any assessment conducted whilst having an infection is unethical practice.
Once clear of infection and classes as medically fit for discharge, the following may be put in place whilst in hospital care;
- Best interest meeting involving betty and multidisciplinary teams
- Re-assessment of medication
- ACE assessment
If Betty is deemed able to return home;
- Referral to CMHT
(Work with landlord, environmental health and OT to assess home environment)
- Referral to Physio to continue in community
- Involvement of diabetic nurse
- Referral to home care team? or alarm system?
- Referral to psychology
(Work toward positive mental health and develop coping strategies)

Possibly suggest to put Betty on a low dose of mirtazapine as she has responded well in the past to stabilise her
Find out what's important to Betty
- Tap into what is important and support Betty to gain trust
- Supporting her with her cats because they're important to her - buying cat food, cat litter

Social work

Mental Capacity Act 2005 sections 2 and 3 for capacity and functioning - From a Mental Health Social Worker perspective this is essential as we need to know how much capacity Betty holds which helps to decide how much input she is able to have in her care.
While Betty is in hospital we could apply for an assessment under section 5 the Mental Health Act to determine her mental state
Skills
Empathy for her need to want to go back home - By empathising with Betty it sort of puts us in her position which emphasises the importance of her returning back home.

Be assertive but honest with her when explaining why she needs to be in hospital - Being honest will help us to build a trustworthy rapport with Betty.
Start to build the relationship with her by introducing myself and explain why i am there - This will offer Betty clarification of the situation as well as making her feel at ease / comfortable.
Use active listening skills to gage why she is so frustrated and what she would like to happen with her care - Active listening will ensure we do not miss anything and will also encourage Betty to talk to us if she knows that we care about what she has to say. Active listening can be demonstrates through body language and responding to sentences with "yes" and questions.

Get her into support groups for hoarders/survivors of IRA troubles
Bereavement counselling - Counselling gives Betty the opportunity to learn about grieving and how to cope emotionally with her loss.

Promote Bettys self determination as she could become overly dependent on services if we do all the work for her
Identify and develop her strengths - By doing so it will enable us to empower her by reassuring her that she has got strengths and can develop some of her needs independently.
Be trustworthy - By building trust with Betty it will help us to build a rapport with her meaning we can gain better insight into her thoughts and feelings.
