Treatments of OCD
Psychological therapy - Exposure Response Therapy (Lehmkuhl)
Jason was a 12 year old boy with both autism and OCD
He had contamination fear, excessive hand-washing, counting, checking
He spent several hours doing his rituals
Jason attended ten 50min CBT sessions over 16 weeks
He was not able to do visualisation techniques so ERP was modified to meet his needs
Jason identified feelings of distress and learnt coping statements
He was exposed to stimuli (door handles and elevator buttons) which caused anxiety, but was prevented from carrying out compulsive behaviour (eg:washing hands)
He had to touch them until he became habituated with it
Results
His Y-BOCS scores dropped from 18 to 3
At 3 month follow-up, he showed improvement in OCD symptoms and social life
Biomedical (SSRIs) (Soomro)
It helps prevent re-uptake of serotonin by the pre-synaptic neurone, to increase levels of serotonin in the synapse for the post-synaptic neurone
Soomro reviewed 17 studies to compare the effectiveness of SSRIs with a placebo
SSRIs group was more effective in reducing symptoms using Y-BOCS 16-13 weeks after treatment
SSRIs reduce severity of OCD because they lessen anxiety associated with it
Psychological therapy - CBT (Lovell)
Aim
To compare effectiveness of CBT delivered via telephone vs via face-to-face
Sample
72 patients from 2 different hospitals
Procedure
Patients were allocated to either group with by randomised control trial
Patients underwent 10 weekly sessions
Changes in symptoms was measured using Y-BOCS, BDI and a client satisfaction questionnaire
Result
6 months post-treatment, the Y-BOCS scores significantly improved in both groups which meant that patients benefit from both forms of CBT
